Privacy Statement

Many people in our community are part of the Armed Forces, whether as serving personnel, reservists, service leavers, veterans, or family members. Military life can bring unique challenges, and if we are able to identify patients who are part of the Armed Forces community, it will help us to offer you the right support. If this applies to you, please let us know by completing the form below.

A code will be added to your medical record to identify you as a member of the Armed Forces community, so practice staff are aware and the surgery may support you with the following:

• all newly registered members of the Armed Forces community will receive a copy of an information pack

• further support to take care of your health and wellbeing, such as priority access to appropriate services (e.g. mental health, social prescribing, health checks)

• with your consent, referrals to specialist Armed Forces or veteran support services

• where available, access to clinics or dedicated support sessions with trained advisers who understand the needs of the Armed Forces community

If your situation changes and you are no longer part of the Armed Forces community, please let us know so we can update your record accordingly. Further information about how we use and process your information can be found on our website.
