The Frailty Team is a non-medical team set up to provide proactive care in care homes, with an emphasis on avoiding unnecessary hospital stays. The team will look at the needs and wishes of residents living in care homes, aiming to prevent crises by planning the future.
Each resident staying permanently in the care home will receive a personalised care plan stating their wishes for the future including decisions around hospital admission, resuscitation status and medical escalation. The Frailty Team work closely with your GP surgery by undertaking a regular planned visit to the care home to assess residents who are feeling unwell on that day. Alongside supporting our care home residents we also offer training and education to care home staff.
